Ramipril is a prodrug that is hydrolyzed in the liver after being absorbed from the gastrointestinal tract to produce an active angiotensin-converting enzyme (ACE) inhibitor ramiprilat, which is a potent and long-acting ACE inhibitor . Taking ramipril will increase plasma renin activity and decrease plasma concentrations of angiotensin II and aldosterone. Because of the decrease in angiotensin II, ACE inhibitors can cause peripheral vasodilation and decrease in vascular resistance, thereby producing beneficial hemodynamic effects. There is evidence that tissue ACE, especially the vascular system rather than ACE in the circulation, is the main factor determining hemodynamic effects. Like kininase II, angiotensin converting enzyme can also degrade bradykinin. There is evidence that the inhibition of ACE caused by ramiprilat has certain effects on the kallikrein-kinin-prostaglandin system. It is clinically used for patients with primary hypertension, congestive heart failure, renal hypertension, and congestive heart failure symptoms that appear within the first few days after the onset of acute myocardial infarction.
