Lidocaine works as a local anesthetic by reversibly closing sodium channels and blocking action potential transmission across nerve fibers. Sensory nerve fibers are blocked earlier than motor nerve fibers, so low doses of lidocaine can exert selective sensory block. Lidocaine also has antiarrhythmic effect, which belongs to class B antiarrhythmic drugs. It can reduce ventricular rate, shorten action potential time and absolute refractory period, and prolong relative refractory period. Lidocaine acts quickly and for a short time. Adding epine'phrine to lidocaine can reduce systemic absorption rate and thus prolong anesthesia time.
Application:
Lidocaine is a local anesthetic, also known as serocaine, which has replaced procaine in recent years and is widely used in local infiltration anesthesia in cosmetic surgery. Lidocaine blocks nerve excitation and conduction by inhibiting the sodium channel of nerve membrane. Its lipid solubility and protein binding rate are higher than that of procaine, and its ability to penetrate cells is stronger, its effect is faster, its action time is longer, and its action intensity is 4 times that of procaine. Clinical applications include infiltration anesthesia, epidural anesthesia, surface anesthesia (including mucosal anesthesia during thoracoscopy or abdominal surgery), and nerve conduction block. In order to prolong the time of anesthesia and reduce lidocaine poisoning and other side effects, epine'phrine can be added to anesthetics. Lidocaine may also be used to treat premature ventricular beats after acute myocardial infarction, ventricular tachycardia, digitalis poisoning, cardiac surgery, and ventricular arrhythmias caused by cardiac catheters, including premature ventricular beats, ventricular tachycardia, and ventricular fibrillation. Secondly, it is also used for epileptic persistent state with other anticonvulsant drugs ineffective and local or spinal anesthesia. But supraventricular arrhythmias are usually ineffective.
